Transcript Request

Former KHS students who are in need of their transcripts may obtain them through the Guidance Office. In order to protect our students' privacy, we cannot accept verbal requests.

Students and parents who are listed on the transcript may come to the Guidance Office during office hours to make a request. A photo ID is required to process all documentation.

Anyone living out of the area (and therefore unable to make a visit to the school) may submit a letter of request through mail, email, or fax instead. The following information must be included in the letter:

  • Name at time of graduation/withdrawal

  • Birth date

  • Date of graduation/withdrawal

  • The address where the transcripts are to be mailed

Please mail the letter to the school along with a clear copy of your photo ID to:

Kingman High School
Attn: Guidance Office
4182 N. Bank Street
Kingman, AZ 86409

If you prefer, you may also fax or email the letter along with a clear copy of your photo ID to: (928) 692-1507 or Email Wendi Welton

Please note that an official transcript cannot be faxed or emailed. Transcripts will be mailed to the address specified in the letter of request.

There is NO charge for this service.
